Social Deteriminants of Health
Supporting you beyond medical care.
Social determinants of health (SDOH) are non-medical factors that influence health and include things like neighborhood environment, education access, and economic stability. Improving the quality of these factors can improve overall health.
When basic needs like food, housing, and transportation become overwhelming, managing your health care can feel out of reach. We partner with Wyoming 2-1-1 to support our members with needs not covered by health insurance, commonly referred to as Social Deterimants of Health (SDOH).
Understanding Social determinants of health
Your health is influenced by more than doctor visits, prescriptions, and medical treatments. Everyday factors known as SDOH play a major role in your overall well-being. SDOH are the conditions in which people live, work, and interact. These factors can impact your ability to stay healthy and manage your care.
