Coding Clarification – Diagnosis Code Policy 

Thank you for your feedback regarding our Diagnosis Code Policy. We have clarified our language to avoid further confusion. 

Diagnosis Code Policy 

If a provider bills an E&M service on the same date as the preventive medicine service (typically on the same claim), with a Z code as the only diagnosis, that is when the E&M code would be denied.
