Claims Submission and Billing Practices

As a contracted provider, please remember that you are required to submit claims on behalf of our members. Additionally, pass-through billing is strictly prohibited and constitutes a violation of your provider agreement.

We appreciate your continued partnership and commitment to compliance. Please reference the specific contract language below:

  • 3.8 Submit Claims for Covered Services on behalf of the Participant in a timely manner. Upon request, Provider agrees to provide any documentation necessary to support the information contained in the Claim.
  • 3.7 Agree not to submit Claims containing pass-through billing items.

Gold Carding Program Launches Soon to Streamline Prior Authorization

Beginning January 1, 2026, we’re introducing the Gold Carding Program—a new initiative that simplifies the prior authorization process for providers with consistently high approval rates.

This program supports our shared goal of timely access to care and aligns with state legislation focused on reducing administrative burden and enhancing provider-payer collaboration.

Eligibility Highlights:

  • Submit ≥5 prior authorization requests for a CPT/HCPC code within 12 months
  • Maintain ≥90% approval rate for those requests

Gold Carding Benefits:

  • Less paperwork
  • Quicker access to care
  • Greater provider satisfaction
  • Continued emphasis on quality, evidence-based treatment

Next Steps:

Eligible providers will receive formal notification and instructions for submitting advance notifications in place of full prior authorization documentation. Gold Card status will be reviewed periodically for compliance.
