Case management services provide personalized support to members with complex medical needs, chronic conditions, or high-cost treatments. These services involve coordinating care with your health care providers and helping you navigate your health insurance benefits. Our team of nurses, doctors, pharmacists, and licensed clinical social workers can work with you to develop care plans and provide education to improve health outcomes. We do this while providing financial advice to keep health care costs manageable and understandable.

Enrolling in Case Management

Please call 1-866-204-7132 or email PopulationHealth@bcbswy.com to see if you qualify for case management services. Once this determination has been made by the Population Health team, qualifying members will enroll in the Case Management program.

An essential part of Case Management is coordination of care. This involves contacting multiple medical providers and resources on your behalf and case managers need permission to do this. The link to the enrollment/consent forms are found in the quick links to the right.
