How to Grant Authorizations

Each person's health information is personal and sensitive and protecting this information is extremely important. Federal laws govern who has access to personal information and how it is used. Therefore, permission is required before any of your protected health information can be released to someone other than you. 

What is Authorization?
Authorization is giving permission for someone to access and manage your personal health information. 
Authorized users can:

  • Access and manage your information on the phone
  • Act on your behalf when they contact Member Services

You can grant authorization to others by completing the HIPAA Authorization to Release Information form.  A covered family member age 18 or older will also need to complete the HIPAA Authorization to Release Information form to grant others permission to access their personal health information.
